Purchases of Penumbra products in North America are subject to the standard Terms and Conditions set forth below.

Terms and Conditions (Effective September 23, 2025)

Returns Merchandise
Permission to return merchandise is required for all returns. Contact the Penumbra Return Department at 1.510.748.3200 or e-mail [email protected] for a Return Merchandise Authorization (RMA) number. All requests for permission to return merchandise should include the reason for the return, the order number and the Penumbra invoice number.

Return all medical devices in their original box with sufficient packing material to ensure safe transit. Product with added writing or labels, packaging damaged (including during return shipping) or defaced with RFID tags or other unremovable stickers, is subject to a 15% reprocessing charge.

The RMA number must be clearly marked in the “Attn:” line of the address on the shipping label. Merchandise returned without an RMA number will not be accepted or considered returned for purposes of the warranty.

Compliance with Anti-Kickback Statute
Amounts billed on Penumbra invoices may reflect discounts or volume rebates on the products listed. In the case of rebates, Penumbra will provide purchaser with a notice after the end of each period for which a rebate is paid that identifies the total purchase price of all products purchased during the period, how the rebate was calculated and the amount of any rebate earned. In the case of both discounts and rebates, Purchaser is obligated to (a) fully and accurately disclose the cost of all products purchased – including any rebates, discounts or other price reductions – in cost reports or claims for reimbursement by purchaser to Medicare, Medicaid, or other health care programs requiring such disclosure, and (b) provide such documentation to representatives of the Secretary of the Department of Health and Human Services and state agencies upon request. This notice is provided in order to comply with the discount safe harbor to the federal Anti-kickback Statute, 42 C.F.R.1001.952(h).
